Cremation Requirements for Fetal Deaths
—–Under 20 weeks gestation, we need:
•Cremation authorization form
•Medical Document stating gestational age*
—–At 20 weeks gestation (or 350 grams) and over -including stillborns, we need:
•Cremation authorization form
•Medical Document stating gestational age*
•Permit
•Medical Examiner Authorization*The Medical Document provided from the hospital or obstetrical department should include: the date and time of procedure/termination, the mother’s name, location of the procedure, length of gestation, and weight of fetus.
Cremation Requirements for Fetal Abortions/Terminations
—–Under 20 weeks gestation, we need:
•Cremation authorization form
•Medical Document stating gestational age & that it was a termination*
—–At 20 weeks gestation (or 350 grams) and over, we need:
•Cremation authorization form
•Medical Document stating gestational age & that it was a termination*
•Medical Examiner Authorization
•(no permit needed)*The Medical Document provided from the hospital or obstetrical department should include: the date and time of procedure/termination, the mother’s name, location of the procedure, length of gestation, and weight of fetus.
Decedents from Connecticut require the following CT forms:
•Burial Transit Removal Permit (VS-9)
•Cremation Permit (VS-48)
•Cremation Certificate (Medical Examiner) (VS-47)
